What gets me is that Trump blamed the media for "making Melania afraid he had a heart attack"... If this was a planned appearance at the hospital, you would think his wife would have known his plan for the day. Most wives ask their spouse what they have planned for the week. Any assistant on staff surely would have thought to call her before CNN reported it. Perhaps from the motorcade before it arrived at the hospital.

My theory for the hospital cost is that Trump is trying to create a narrative that gives Trump an excuse as to why he either, chooses to not seek re-election, or allows him to drop out of the race if it becomes clear he won't win. Think about it, I think him referring to the hospital visit as phase 1, and will continue next year. I'm certain he will say he has some medical problem that makes him unable to campaign and be president. Phase 1 is sowing the seed of the narrative, then using it if and when its clear he needs an excuse for losing. Trump is way too vain to allow someone to beat him and will always be able to say "if I had been able to run again I would have won for sure, if not for my condition."

In the military we have broken phases of physicals. Blood work and immunizations, detailed health report, and then any remedies necessary. So I wouldn't say its unlikely that he could have split his physical into phases.
